CLUB MEMBERSHIP AGREEMENT:

This club membership agreement ("this agreement") is entered into between LBOH Social Club ("the company") and the member named below ("Member") and incorporates the Terms and Conditions and Rules stipulated by the Company and its representatives. This Agreement is the exclusive agreement between the Company and Member and Member affirms that Member has not relied upon any representatives or warranties not contained herein.

The Company agrees to provide Member with membership to the Company's club known as LBOH Social Club. (Including any facilities or events to which access provided in connection therewith the "Club") The Member agrees to pay for such membership and otherwise abide by the terms set forth herein (including the Terms and Conditions and any Rules)

Release of Liability:

By signing this agreement, the Member agrees to indemnify and hold harmless the Company from any and all claims, losses, liabilities, damages, and expenses (including attorney fees) arising out of or related to the Client's use of the social club facilities.

MEMBER DETAILS:

TERMS & CONDITIONS:

1. Membership to the Social Club is dependent on acceptance through an interview process. Once membership form is received, you will be contacted for an interview.

2. Annual membership for members is $600. This can be billed monthly upon request. (Credit Card or ACH Information will be required to be on file for monthly billing)

3. 1 person/membership access to the club plus guest pass.

4. Weekly Open House on Monday's from 5:00-10:00 PM for all members. Club managers will inform member of any changes 1 week prior.

5. Once membership is completed, member will receive key fob to building.

6. Member will be assigned a personal locker located in the building.

CLUB RULES:

• Any act of violence or threats of violence will result in permanent loss of membership or a ban from the club.

• No abusive, vulgar or excessive profanity.

• Sexual harassment and or sexually inappropriate conduct will not be tolerated.

• Use or possession of drugs on the premises is strictly forbidden.

• No weapons of any kind are allowed in the club.

• Destruction or intentional damage of club property or the property of any person is prohibited and will result in replacement fees.

• Smoking is only allowed in designated smoking areas.

• Only legitimate trained service dogs on a leash are permitted inside the club. Therapy or emotional support animals are not allowed.

• Children under the age of 16 must be accompanied by an adult.

• Everyone is expected to share in the responsibilities of helping to keep the club clean. Discarding waste or recycling in the appropriate bins.

KEY FOB ASSIGNMENT & REPLACEMENT:

Members will be assigned a key fob once membership is completed. A new member packet will be shipped to the members address listed above which will contain the key fob and other club items. Members are responsible for keeping track of the fob. Loss or damage of the key fob will result in a $25 replacement fee.

LOCKER ASSIGNMENT:

Members will be assigned a locker once membership is completed. Lockers are for members personal belongings inside the club. Members are responsible for keeping track of the key to the locker. Loss or damage of the locker key will result in a $25 replacement fee.
